    Eduard Gallyamov – PhD, professor, honored doctor of the Russian Federation, head of the general surgery department of the faculty of general medicine of FSAEI HE I.M. Sechenov First MSMU MOH Russia (Sechenov University) has shared his experience with Irkutsk oncologists. Moreover, this event took place in the operating room of the center.

    “When we invite people who are experts in certain medical technologies – such as Eduard Gallyamov, who has mastered all types of endoscopic surgery, it is always very interesting and important,” emphasizes MD, professor Victoria Dvornichenko. “Of course, you can master all this on your own – but much slowler and with flaws that can deplorably affect patients’ health.”

    On the first day of the master class, two radical prostatectomies (prostate removal) were performed. Such operations are regularly performed by urologists of the oncological hospital, but, nevertheless, they have learned some new subtleties and details. In addition, one of the patients was quite ‘difficult’: with a high body mass index or, to put it simply, obese. The second patient had not only the prostate, but also the kidney (nephrectomy), affected by cancer removed. Thus, two operations were performed at the same time, saving this elderly person from having to go to the operating table twice and, accordingly, going twice through a complex procedure of postoperative nursing and recovery. By the next day the patient was able to walk on his own – this is one of the undoubted advantages of laparoscopic operations.

    It should be noted that ‘inconvenient’ patients, who can cause difficulties for doctors were specially selected for the master class. Therefore, on the second day of the master class, a patient in the twentieth week of pregnancy was selected: she underwent adrenalectomy (removal of the adrenal gland). So, the doctors worked very ‘delicately’ so as not to harm either the woman or her fetus. Suffice to say that blood loss during this operation was only 2 milliliters of blood!

    Another operation on the second day of the master class was a resection of the 7th segment of liver. It was performed for the first time in the Irkutsk region not by opening the abdominal cavity, but laparoscopically, that is, through a small incision. The next innovation was a radical cystectomy (removal of the bladder). Usually, during such operations, both ureters are diverted to the skin, which is very uncomfortable for patients. However, the patient operated under the guidance of Eduard Gallyamov, did not have to face such inconvenience. The ureters were sewn into a ‘reservoir’ from the patient's small intestine, and then this reservoir was placed to the anterior abdominal wall. Prior to this, similar operations have not been performed in the Angara region.

    In addition, the doctors performed an organ-preserving operation to remove a tumor from the right kidney - the only one the patient has! The operation was done without clamping the renal blood flow, in conditions of zero ischemia - so as not to disrupt the organ function. Once again, it should be emphasized that all the operations were performed laparoscopically, that is, sparing for the patient – they were performed through small incisions and punctures.

    “It was very convenient to work with the doctors of the Irkutsk Cancer Center, whose high professionalism provided us with complete understanding,” Eduard Gallyamov shares his impressions. “I also would like to note that the operating room where we worked was packed with the most modern equipment. 3D-imaging and intraoperative ultrasound systems (used for navigation and clarification for the object localization, searching for non-palpable foci, assessing the prevalence of the tumor process, conducting constant monitoring of manipulations, clarifying the radicality of the operation performed and the passability of vascular reconstructions) and much more - in a word, not every operating room where I have worked was equipped with such a powerful set of the latest equipment.”

    “With the money allocated within the framework of the federal project ‘Fighting Cancer Diseases’, we purchased and updated a large amount of equipment, including endoscopic,” explains Victoria Dvornichenko. “So, we have a great interest in ensuring that this equipment brings maximum benefit to our patients. Therefore, not only we learned from Eduard Gallyamov live: videos of operations performed during the master class will become an invaluable educational material for our doctors.”
